    Fatal myocardial infarction after lung resection in a patient with prophylactic preoperative coronary stenting†

    In this report we present the case of a 77-yr-old man who underwent resection of the upper lobe of the left lung for a carcinoma, six weeks after percutaneous transluminal coronary angioplasty (PTCA) with stenting of the left anterior descending (LAD) and circumflex coronary arteries. Antiplatelet therapy with clopidogrel was interrupted two weeks before surgery to allow for epidural catheter placement and to minimize haemorrhage. The surgical procedure was uneventful. In the immediate postoperative period, however, the patient suffered severe myocardial ischaemia. Emergency coronary angiography showed complete thrombotic occlusion of the LAD stent. In spite of successful recanalization, reinfarction occurred and the patient died in cardiogenic shock. Prophylactic preoperative coronary stenting may put the patient at risk of stent thrombosis if surgery cannot be postponed for three months. In such cases, other strategies such as perioperative β-blockade for preoperative cardiac management should be considered. Br J Anaesth 2004; 92: 743-

    NETQOS policy management architecture for flexible QOS provisioning in Future Internet

    This paper is focussed on the NETQOS architecture for automated QoS policy provisioning, which can be used in Future Internet scenarios by the different actors (i.e. network operators, service providers, and users) for flexible QoS configuration over combinations of mobile, fixed, sensor and broadcast networks. The NETQOS policy management architecture opens the possibility to specify QoS policies on a "business" level using ontology descriptions and policy management interfaces, which are specific to the actors. The business level policy specifications are translated by the NETQOS system into intermediate and operational QoS policies for automated QoS configuration at the managed heterogeneous network and transport entities. NETQOS allows QoS policy specification and dependency analysis considering Service Level Agreements (SLAs) between the actors, as well as automated policy provisioning and adaptation. The interaction of the NETQOS components is based on a common po licy repository. The particular focus of the paper is aimed to discuss ontology and actor oriented QoS policy specification and configuration for heterogeneous networks, as well as NETQOS QoS policy management interfaces at business level and automated translation of business QoS policies to intermediate and operational policy level

    Predicting consumer biomass, size-structure, production, catch potential, responses to fishing and associated uncertainties in the world's marine ecosystems

    Existing estimates of fish and consumer biomass in the world’s oceans are disparate. This creates uncertainty about the roles of fish and other consumers in biogeochemical cycles and ecosystem processes, the extent of human and environmental impacts and fishery potential. We develop and use a size-based macroecological model to assess the effects of parameter uncertainty on predicted consumer biomass, production and distribution. Resulting uncertainty is large (e.g. median global biomass 4.9 billion tonnes for consumers weighing 1 g to 1000 kg; 50% uncertainty intervals of 2 to 10.4 billion tonnes; 90% uncertainty intervals of 0.3 to 26.1 billion tonnes) and driven primarily by uncertainty in trophic transfer efficiency and its relationship with predator-prey body mass ratios. Even the upper uncertainty intervals for global predictions of consumer biomass demonstrate the remarkable scarcity of marine consumers, with less than one part in 30 million by volume of the global oceans comprising tissue of macroscopic animals. Thus the apparently high densities of marine life seen in surface and coastal waters and frequently visited abundance hotspots will likely give many in society a false impression of the abundance of marine animals. Unexploited baseline biomass predictions from the simple macroecological model were used to calibrate a more complex size- and trait-based model to estimate fisheries yield and impacts. Yields are highly dependent on baseline biomass and fisheries selectivity. Predicted global sustainable fisheries yield increases ≈4 fold when smaller individuals (< 20 cm from species of maximum mass < 1kg) are targeted in all oceans, but the predicted yields would rarely be accessible in practice and this fishing strategy leads to the collapse of larger species if fishing mortality rates on different size classes cannot be decoupled. Our analyses show that models with minimal parameter demands that are based on a few established ecological principles can support equitable analysis and comparison of diverse ecosystems. The analyses provide insights into the effects of parameter uncertainty on global biomass and production estimates, which have yet to be achieved with complex models, and will therefore help to highlight priorities for future research and data collection. However, the focus on simple model structures and global processes means that non-phytoplankton primary production and several groups, structures and processes of ecological and conservation interest are not represented. Consequently, our simple models become increasingly less useful than more complex alternatives when addressing questions about food web structure and function, biodiversity, resilience and human impacts at smaller scales and for areas closer to coasts

Para tanto, foram utilizadas remedi\ue7\uf5es em intervalo de 7 anos de 251 indiv\uedduos de arauc\ue1ria ocorrentes nas parcelas permanentes do Projeto PELD/CNPq. Foi constru\ueddo um modelo de produ\ue7\ue3o, tendo como vari\ue1vel dependente o di\ue2metro futuro, utilizando regress\ue3o e an\ue1lise de covari\ue2ncia. Como vari\ue1veis independentes, foram utilizadas as caracter\uedsticas dimensionais e sociol\uf3gicas dos indiv\uedduos e os \uedndices de concorr\ueancia independentes de dist\ue2ncia. A vari\ue1vel mais correlacionada com o di\ue2metro futuro foi o di\ue2metro atual. Os \uedndices de concorr\ueancia apresentaram alta correla\ue7\ue3o com a vari\ue1vel dependente, representando a press\ue3o competitiva exercida sobre a \ue1rvore, mas n\ue3o foram selecionados no modelo de regress\ue3o. Somente o di\ue2metro atual, multiplicado por um coeficiente, acrescido de um valor constante o qual diferiu de acordo com a posi\ue7\ue3o sociol\uf3gica do indiv\uedduo, foi eficiente para predizer o di\ue2metro futuro. Foram constru\ueddos tr\ueas varia\ue7\uf5es do modelo de produ\ue7\ue3o. Esses modelos apresentaram melhores estimativas para indiv\uedduos de Araucaria angustifolia emergentes, no entanto, o seu emprego na prognose de crescimento de \ue1rvores individuais deve ser cauteloso por causa da magnitude dos erros de predi\ue7\ue3o que apresentou.This work aims to model the diameter growth for individual trees of Araucaria angustifolia , present in the Mixed Ombrophylous Forest in the northeast of Rio Grande do Sul. Thus, 251 araucaria individuals located on the permanent plots of the Project PELD \u2013 CNPq were remeasured after a period of 8 years. A yield model was built, having as dependent variable the future diameter, using regression and analysis of covariance. As independent variables, the dimensional and social characteristics of the individuals and distance independent competition indices were used. The most correlated variable with the future diameter was the current diameter. The competition indices showed high correlation with the dependent variable, representing the competitive pressure exerted on the tree, but they were not selected in the regression model. Only the current diameter, multiplied by a coefficient, plus a constant value which differed according to the sociological position of the individuals was effective to predict the future diameter. Three variations of the production model were built. The models showed better estimates for emerging individuals of Araucaria angustifolia, however, its use in the individual tree growth prognosis should be cautious due to the magnitude of the errors it presented

    The sex of an individual is determined by the fate of the gonad. This organ arises from two different structures: the coelomic epithelium and a mesenchymal part that forms from the mesonephros. The early embryonic gonad can differentiate into a testis or an ovary, thus suggesting that at an early stage the gonad is bipotential. Testis formation requires differentiation of Sertoli cells, which will form the supporting cell lineage of the seminiferous tubules. These cells synthesize Anti-Mullerian Hormone (AMH), which induces regression of the Mullerian duct, thus counteracting the development of female internal genitalia. Moreover, Sertoli cells favor recruitment of other somatic cell lineages migrating from the mesonephros that are also crucial for testis development to occur. The interstitial area of the testis contains the steroidogenic cells (Leydig cells), which have the function of producing androgens. These hormones stimulate the differentiation of internal and external genitalia of the male [for a review see Gonad differentiation depends on the paternal transmission of the sex chromosome. Thus, an XY embryo develops as a male, whereas an XX embryo becomes female. Most of the genes involved in this developmental pathway have been discovered from genetic studies of human XY sex-reversal. At a molecular level, the Y chromosome encodes a testis determining factor, SRY [Sinclair et al., Key Words Abstract In mammals, the sex of the embryo is determined during development by its commitment either to the male or female genetic program regulating testicular or ovarian organogenesis. Major steps towards unraveling sex determination in mammals are achieved by the identification of key genes involved in human pathologies and the application of mouse genetics to analyze their function. While the expression of Sry and Sox9 is sufficient to induce the male developmental program, the molecular pathways that specify ovarian differentiation were unclear before the recent demonstration that mutations in the RSPO1 gene induce femaleto-male sex reversal in XX patients. By generating the corresponding mouse model, we have shown that Rspo1 is so far the earliest known gene controlling the female genetic developmental program. Rspo1 activates the canonical ␤ -catenin signaling pathway required for female somatic cell differentiation and germ cell commitment into meiosis. The aim of this review is to describe the roles of R-spondins (Rspo) in developmental processes and disorders and the current knowledge obtained from murine models.     Dissection of the Complex Phenotype in Cuticular Mutants of Arabidopsis Reveals a Role of SERRATE as a Mediator

    Mutations in LACERATA (LCR), FIDDLEHEAD (FDH), and BODYGUARD (BDG) cause a complex developmental syndrome that is consistent with an important role for these Arabidopsis genes in cuticle biogenesis. The genesis of their pleiotropic phenotypes is, however, poorly understood. We provide evidence that neither distorted depositions of cutin, nor deficiencies in the chemical composition of cuticular lipids, account for these features, instead suggesting that the mutants alleviate the functional disorder of the cuticle by reinforcing their defenses. To better understand how plants adapt to these mutations, we performed a genome-wide gene expression analysis. We found that apparent compensatory transcriptional responses in these mutants involve the induction of wax, cutin, cell wall, and defense genes. To gain greater insight into the mechanism by which cuticular mutations trigger this response in the plants, we performed an overlap meta-analysis, which is termed MASTA (MicroArray overlap Search Tool and Analysis), of differentially expressed genes. This suggested that different cell integrity pathways are recruited in cesA cellulose synthase and cuticular mutants. Using MASTA for an in silico suppressor/enhancer screen, we identified SERRATE (SE), which encodes a protein of RNA–processing multi-protein complexes, as a likely enhancer. In confirmation of this notion, the se lcr and se bdg double mutants eradicate severe leaf deformations as well as the organ fusions that are typical of lcr and bdg and other cuticular mutants. Also, lcr does not confer resistance to Botrytis cinerea in a se mutant background. We propose that there is a role for SERRATE-mediated RNA signaling in the cuticle integrity pathway

    Hemodynamics optimization during off-pump coronary artery bypass: the 'no compression' technique

    OBJECTIVE: Heart manipulation during OPCAB may cause hemodynamical instability in particular for access to the posterior and lateral walls. The 'no compression' technique involves enucleation of the heart without any compression on the cavities, and stabilization of the target area with a suction device. The impact of this technique on hemodynamics is assessed. METHODS: In order to analyze a homogeneous group, 26 consecutive patients with triple grafts, one to each side of the heart in the same sequential order (posterior, lateral and anterior wall successively) were selected. Heart rate (HR), mean pulmonary arterial pressure (PAP, mmHg), pulmonary capillary wedge pressure (PCWP, mmHg), mean arterial pressure (MAP, mmHg), cardiac output index (COI, l/min per m(2)), and central venous saturation (SvO(2),%) were monitored. A coronary shunt was used for all the anastomoses. RESULTS: HR was stable with baseline value of 60+/-10 and the highest value for the anterior wall, 63.6+/-8 (P=0.23). PAP and PCWP exhibited their highest increase, when compared with baseline, for the lateral wall, 23.9+/-4.7 vs. 20.7+/-6.2 (P=0.06), and 17.2+/-4.7 vs. 14.9+/-5.6 (P=0.16), respectively. MAP, COI and SvO(2), exhibited their largest drop, when compared with baseline, for the lateral wall too, 73.1+/-9.1 vs. 77.1+/-7.5 (P=0.12), 1.99+/-0.47 vs. 2.26+/-0.55 (P=0.09), and 70.5+/-8.4 vs. 74.8+/-9.3 (P=0.12), respectively. CONCLUSIONS: None of the hemodynamical parameter differed significantly from baseline value for all three territories. While hemodynamics was perfectly maintained during the posterior and anterior walls revascularization, exposure of the lateral wall led to marginal changes only

    Um das Verhalten von Zwischenkulturen bei verschiedenen Umweltbedingungen besser zu verstehen, wurde ein Netzwerk von Versuchsparzellen entlang der Achse Genf- Zürich und im Jura eingerichtet. Einige der untersuchten Arten vermochten den Boden im Herbst schnell zu bedecken (z.B. Sareptasenf). Andere Arten produzierten eine bedeutende Biomasse oberirdischer Pflanzenteile (z.B. Sonnenblume). Wieder andere, im Herbst weniger leistungsfähige Arten, stellten eine gute Bodenbedeckung Ende Winter sicher, wie der Rauhafer. Mit einer multifaktoriellen Analyse konnten die Beziehungen zwischen den Leistungen der verschiedenen Zwischenkulturen und den Umweltbedingungen untersucht werden. Keine Art vereinigt alle über die gesamte Zwischenkultur erwünschten Eigenschaften, Mischungen verschiedener Arten sind jedoch vielversprechend

    A Permeable Cuticle Is Associated with the Release of Reactive Oxygen Species and Induction of Innate Immunity

    Wounded leaves of Arabidopsis thaliana show transient immunity to Botrytis cinerea, the causal agent of grey mould. Using a fluorescent probe, histological staining and a luminol assay, we now show that reactive oxygen species (ROS), including H2O2 and O2−, are produced within minutes after wounding. ROS are formed in the absence of the enzymes Atrboh D and F and can be prevented by diphenylene iodonium (DPI) or catalase. H2O2 was shown to protect plants upon exogenous application. ROS accumulation and resistance to B. cinerea were abolished when wounded leaves were incubated under dry conditions, an effect that was found to depend on abscisic acid (ABA). Accordingly, ABA biosynthesis mutants (aba2 and aba3) were still fully resistant under dry conditions even without wounding. Under dry conditions, wounded plants contained higher ABA levels and displayed enhanced expression of ABA-dependent and ABA-reporter genes. Mutants impaired in cutin synthesis such as bdg and lacs2.3 are already known to display a high level of resistance to B. cinerea and were found to produce ROS even when leaves were not wounded. An increased permeability of the cuticle and enhanced ROS production were detected in aba2 and aba3 mutants as described for bdg and lacs2.3. Moreover, leaf surfaces treated with cutinase produced ROS and became more protected to B. cinerea. Thus, increased permeability of the cuticle is strongly linked with ROS formation and resistance to B. cinerea. The amount of oxalic acid, an inhibitor of ROS secreted by B. cinerea could be reduced using plants over expressing a fungal oxalate decarboxylase of Trametes versicolor. Infection of such plants resulted in a faster ROS accumulation and resistance to B. cinerea than that observed in untransformed controls, demonstrating the importance of fungal suppression of ROS formation by oxalic acid. Thus, changes in the diffusive properties of the cuticle are linked with the induction ROS and attending innate defenses
